CJN Therapy is an in-network provider for many insurance companies, including BCBS, United Healthcare, Cigna, and Aetna.  I may also be considered an out-of-network provider for some insurance companies. As a professional courtesy, insurance coverage will be verified prior to the start of therapy and insurance claims will be filed for you.  However, you may still be responsible for some or all of the fee for therapy.  A quote of benefits is not a guarantee of payment, and it is the policyholder's responsibility to understand insurance benefits. It is also the client's responsibility to notify me if there are any insurance updates/changes during the course of treatment. Failure to notify of changes may result in the client's financial responsibility for any denied claims due to lapse/change in coverage. 

I also offer the option to self-pay for clients who do not have insurance coverage or prefer to not use their insurance coverage. 

Billing rates and codes:

90791 (intake) - 160.00

90837 (53+ minutes of therapy) - 160.00

90847 (family therapy with the client present) - 160.00

90834 (individual therapy 38-53 minutes) - 140.00

No show fee (not billable to insurance): $80.00

Clients are required to keep an active method of payment on file for billing purposes. I use IvyPay to process payments. If you prefer to use a different payment method this must be discussed at the first appointment. 

I am in compliance with the No Surprises Act and will provide all self-pay clients with a Good Faith Estimate of your fee. The Good Faith Estimate reflects the cost of the services you are seeking at our first appointment. Additional estimates will be available upon request.

For clients with Cigna, Aetna/Aetna State of IL plans and United Healthcare/UMR/United Behavioral Health plans:

   Effective 1/2023, all billing for Aetna, Cigna, and United clients is completed through a company called Alma. I am still a covered provider for these insurance plans, but any insurance/billing questions are fielded through Alma. Feel free to ask me any questions you may have about this service!
